Enroll in PediPRN

It is preferred that practices enroll in PediPRN before using the free service. If you are not enrolled at the time of the call, the care coordinator will assist in getting your practice enrolled at that time. PediPRN services are available from 8:30 a.m. to 5 p.m. Monday through Friday, except for holidays. Enroll online using the form below. To enroll by mail, download PDF versions of both the enrollment form and pre-enrollment survey to enroll by mail. Pediatric primary care physicians are requested to complete both forms.
